What is circumcision?

The sleeve of skin and mucosal tissue that normally covers the glans (head) of the penis is surgically removed. The foreskin is a double layer that is sometimes referred to as the prepuce.

What is the foreskin there for?
When erect, the foreskin provides ample loose skin for the penis to occupy. It’s a movable skin sheath for the penis that reduces chafing and the need for artificial lubricants while also allowing the glans and foreskin to naturally stimulate each other during intercourse.

What are some reasons that circumcision is performed?

1. Hygiene – A circumcised penis is often said to be cleaner or easier to keep clean. When the foreskin is present, smegma (a natural substance made up of dead skin cells, normal flora, and secretions containing the natural antibacterial agent lysozyme) is more likely to accumulate.

2. Infections – Lower risk of urinary tract infections (UTIs); lower risk of penile cancer; lower risk of cervical cancer in partners; lower risk of sexually transmitted diseases (STD).

3. Circumcision is common in many cultures and religions for a variety of reasons.

There are many surgical treatment options available:

1. Laser circumcision.

A laser device is used to cut the skin during laser circumcision. Early recovery and minimal blood loss are two advantages. The cuts on the skin are more precise than traditional circumcision methods. Sutures dissolve on their own after a few days. The amount of dressing needed is minimal. The number of follow-ups required is kept to a bare minimum. This is by far the most cutting-edge circumcision treatment available.

The procedure has no discernible side effects. However, one must accept that, like any other surgical procedure, circumcision has risks and side effects.

2. Aanstomat or Stapler circumcision

The surgery is performed with the use of a stapler device. The procedure is most commonly performed with a second-generation ZSR device. To remove the foreskin, a stapler circumcision device is placed over the penis.

Following the procedure, you will be left with a ring of small metal clips that will fall off on their own after a few days. While the rings are in place, they may cause irritation or pain, which can be easily alleviated with painkillers.

A third-generation device that uses a silicone ring instead of metal clips is also available; silicone rings are used to hold the cut layers of the skin together. The silicone ring naturally falls off after a few days.

3. Silicone ring

This is one of the more recent circumcision methods. It is, however, not as widely used as other methods. The main reason is that it takes several days for the silicone ring to fall off, making it difficult to care for the wound during that time.

Is Circumcision Surgery painful?

It is more uncomfortable than painful. Nothing, however, was severe or unbearable. You will be given enough painkillers and anti-inflammatory medications after surgery to cover any potential pain and swelling.

What type of anaesthesia is used in Circumcision Surgery?

In most cases, the procedure is performed under local anaesthesia. This is the safest and most appropriate type of anaesthesia for this procedure. However, an experienced anaesthetist’s opinion is always sought before deciding on the best method of anaesthesia for you.

Will I require hospital admission?

It is usually done as a day procedure, with at least a 3-hour hospital stay recommended. If you have co-morbidities, your stay may be extended if you require extensive anaesthesia or additional tests. We can safely say, however, that admission times rarely exceed 24 hours.

What investigations do I need to do before surgery?

Blood tests are performed on a regular basis to ensure that your procedure is planned safely. Prior to the procedure, you will be given all specific instructions regarding investigations and pre-operative preparations.
